Hospital Satisfaction Scale with Email Triggers

In this survey, we offer a satisfaction scale (1 to 5) which can be used for any industry. Currently it is a hospital followup example. We have setup email triggers based upon conditions... so at the completion of the survey an email will arrive depending on the answer to the question. In order to have this working correctly you must be using a private hosted CHECKBOX trial or the trial of CHECKBOX Survey Server... and have email options turned on

Directions:
1. Extract and save the contents of the zip file to your desktop
2. In CHECKBOX, at the Survey Manager view, import the file from XML.
3. This will not copy styles or images, just the survey design and logic.

Don't forget to add a style and customize the look and feel.
