Complex template: mission impossible!

We are California State Unviersity, Fullerton. I am going to buy CheckBox. However, I found that checkbox can't support complex template as header and footer and centre all pages.

Please help me to create one complex style and I can do the rest. Please see the attachment file. I have create the htm file.

I edited your post since you don't want a user name and password for your installation on a publicly accessible forum.

Are you currently working with a Prezza Sales associate? If so, please contact that person directly and they will be sure find the solution for you.

One of our guys checked out your install, and made a template for you called Cal State Fancy. It has a CalState logo and is a centered format. We also made it your default style (which you can change if you want).

Hi,

I got the quote last week and we currently evaluate. However, we stuck with the template. Thanks that you already create for us a calstate fancy template. However, it is not the same as our requirement.

1. You can check out the test survey that use your template: https://campusapps2.fullerton.edu/su...?ForceNew=true
(the survey content is out side the box).

2. We need more complex than your sample template. California State University, Fullerton. We have header and footer. They align in the page center. We have complex header rather than logo.

3. We have the body of the survey also in the center of the screen as well. I can't do that because we can't customize the body of the survey.
