First, let me apologize for the late reply. We had a problem with the forum for over a month that prevented thread notifications from being mailed to our support staff and we just found about it this week.

There isn't a built-in option to center the survey, though newer versions Checkbox allow you to create HTML templates to apply to pages of the survey. You could take advantage of this feature to create a custom template that centers the survey in the browser.

Default formatting surveys like your demos

I could not figure out how you format your demo surveys with rounded corners, centered text etc. I just need to know on how to go about applying a template to the surveys.

I'm glad to hear that you figured it out. The two most common techniques used are to use the style header/footer to wrap the survey page with centering text. Another technique is to use the item position options within the survey editor to center items. A third technique would be to create custom page layout templates to apply to the survey.
