Uploaded Image Doesn't Appear

We recently purchased the survey server and I am trying to load an image into the header and into the footer of my style. I follow the image upload process to browse to my local machine. When I click the Preview button, instead of seeing my image I see the "broken image" type of graphic. The path listed below it is correct...it just isn't seeing my image. (It isn't seeing my image even when I continue with the upload.)

Any ideas what I should check to get this to work correctly?

If you go to Settings->Application Settings, is the "Store Images in Database" checked?

Thanks for the reply - that setting was checked correctly. We figured out that it was something about how the image was saved (Macs can save images with a preview or something such as that?) and we got it to work now.
