support@prezzatech.com. I have also attached 5 of the images. Let me know if I should send them somewhere else that you may access. I am sending several images: one of the actual display to the users, the summary on the admin site, and the actual responses for each of the questions. Let me know if you need to see something else, this matter is critical.
