I got the quote last week and we currently evaluate. However, we stuck with the template. Thanks that you already create for us a calstate fancy template. However, it is not the same as our requirement.
1. You can check out the test survey that use your template:

2. We need more complex than your sample template.
California State University, Fullerton. We have header and footer. They align in the page center. We have complex header rather than logo.
3. We have the body of the survey also in the center of the screen as well. I can't do that because we can't customize the body of the survey.
